Immunization records printed from this site may not be complete. The records represent only the data reported to and entered in the system.
If you are experiencing trouble retrieving an immunization record, it is likely due to missing security information on the account you are attempting to access. Please contact the COVID-19 Helpline at 1.800.401.0946 or NV WebIZ Help Desk at IZIT@health.nv.gov for further assistance. You will be required to verify your identity and provide either a cell phone (with texting enabled) or an email address to be added to the record- this information must be saved on the record you are attempting to retrieve in order to use the Portal. You must have access to the cell phone or email as a verification code will be provided through the option you have selected.
